LETTER OF AGREEMENT

The Dana College Social Work Program and____________________________________
hereby agree that the agency will provide field instruction for social
work
students according to the mutually agreed upon guidelines
listed below.
The agency shall determine the number of students which it is prepared
to accept at any given time and shall have the opportunity to interview
students recommended by the college to determine their suitability
for a field practicum experience in the agency.
Field instructors provided by the agency and the practicum coordinator
of the college will collaborate in developing methods of instruction
to implement the objectives of the practicum as stated in the
course syllabus. The field instructors will provide regular supervisory
conferences with the students on at least a weekly basis and
will
be responsible for written evaluations of the students' progress
at the end of the practicum. Grades will be assigned by the college
field coordinator in consultation with the agency and the student.
The college will provide consultation by the practicum coordinator,
and will provide ongoing training opportunities for field instructors.
This training will be specific to the needs of the bachelor’s
level social work student and their supervisors.
The agency will retain full responsibility for its clients
and/or patients and will maintain administrative and professional
supervision
of the students insofar as their presence affects the operation
of the agency and/or the direct or indirect provision of
services to
its clients and/or patients.
The agency shall be responsible for arranging immediate care
in case of accident or illness of the student but is not
responsible for
the costs involved, follow-up care, or hospitalization.
It is understood that assigned students are not agency
or college employees and thus are not covered by Social
Security,
Employment
Compensation, or Workmen's Compensation.
The college and the agency do not and will not discriminate
against any applicant for field instruction because
of race, ethnicity,
color, religion, disability, gender, national origin,
age or sexual orientation.
This agreement may be terminated by either party upon
notice to the other three months in advance and is
executed in
duplicate by the
authorized officers of both parties.
