Note to the student: This evaluation is designed to help you gain
increased skill in self-assessment and assessment of your experiences.
Its aim includes developing awareness of your progress toward competence
in the major areas of professional social work as well as providing
feedback on your experience in the practicum. A thoughtful completion
of this evaluation will be a useful tool in your continued development
as a professional social worker.
1. Summarize the learning experiences in which you participated
during your field practicum.
2. Describe your teaching/learning relationship with your field
instructor. What did you learn from the experience of working under
supervision? (Include any difficulties you had in learning to accept
feedback on your performance.)
3. Describe you working relationships with other agency personnel.
Did you feel accepted as part of the agency? What did you learn from
working as part of an agency staff?
4. To what extent do you feel you understand the purpose, policies,
organization, and activities of your agency? Explain your major
strengths and weaknesses in these areas.
5. To what extent do you feel you understand and feel comfortable
in the community context of your agency? Briefly describe the community,
its values and norms, its racial and ethnic composition, and its
unmet needs. Describe the process by which you learned to work
within the context of this community, particularly with regard
to groups different from your own background.
6. To what extent were you able to develop knowledge of community
resources useful to your clients? What gaps exist in your knowledge?
7. How would you assess your development of a sense of professional
accountability? This includes such areas as confidentiality, recording,
dependability in following through on assignments, etc.
8. To what extent do you feel you are developing a sense of professional
identity? Describe the process during which this has taken place.

9. Describe your progress in developing skills in interviewing and
in forming appropriate relationships with your clients. Please
pay special attention to your growth in attitudes and skills appropriate
to working with diverse racial and ethnic groups, as well as other
diverse populations. Are there areas where you feel especially
strong or lacking in confidence?
10. To what extent do you feel you have been able to develop professional
social work skills in such areas as assessment, development and
implementation of intervention plans, and evaluation? Describe
your strengths and weaknesses.

different levels with individuals, families, groups, and with community
